Carlo Peña, PT, CSCS

Carlo Peña is a licensed Physical Therapist and Certified Strength and Conditioning Specialist (CSCS) with extensive experience in outpatient orthopedics, sports rehabilitation, and cash-based private practice. He specializes in helping athletes and active adults recover from injury, improve performance, and return safely to sport—with a particular focus on sprinting mechanics, deceleration control, and jump-based power training.

Carlo has treated a wide range of orthopedic and post-operative conditions in both clinic and private settings, and has worked with competitive soccer athletes through the MLS Next Academy at Metropolitan Oval. His background includes experience at Motion Sports Medicine, Fit Club Astoria, US Athletic Training Center, and as owner of a private PT practice. He has also rehabilitated Olympic weightlifters and high-performance athletes, integrating strength training with evidence-based rehab.

Carlo is currently completing his Transitional Doctorate in Physical Therapy (tDPT) at the University of Montana and holds a Bachelor of Science in Physical Therapy from the Philippines.

Dr. Cedric Alfred Baldeviso is a licensed Doctor of Physical Therapy with extensive experience in outpatient, subacute, and home-based rehabilitation. He specializes in treating neuromuscular and musculoskeletal conditions using evidence-based, function-focused physical therapy.

He has worked with a wide variety of patients including post-surgical cases, stroke survivors, individuals with chronic pain, orthopedic injuries, balance and gait deficits, and older adults experiencing mobility decline. His treatment approach prioritizes individualized care plans, progressive strengthening, mobility retraining, and patient education to restore independence and improve quality of life.

Alongside his clinical work, Dr. Baldeviso has served as a national board exam lecturer for physical therapy students, teaching anatomy, kinesiology, and advanced rehabilitation concepts. He holds multiple specialty certifications including geriatrics, stroke rehab, Astym soft-tissue therapy, spine care, and cardiac rehabilitation, and is CPR/BLS certified.

Jerome Rivera, PT, DPT

Dr. Jerome T. Rivera Jr. is a licensed Doctor of Physical Therapy specializing in outpatient orthopedic, neurological, cardiopulmonary, and geriatric rehabilitation. He is the founder of NYC Home Rehab Physical Therapy PLLC, a hybrid practice providing one-on-one physical therapy both in-clinic and through mobile in-home visits across New York City.

Dr. Rivera earned his Doctor of Physical Therapy from the University of Montana and graduated cum laude with a Bachelor of Science in Physical Therapy from the University of Santo Tomas in the Philippines. He is licensed in New York and previously practiced in the Philippines, where he gained clinical experience in hospital-based, outpatient, pediatric, and cardiac rehabilitation settings.

Before launching NYC Home Rehab Physical Therapy, Dr. Rivera served as a Director of Physical Therapy in an outpatient medical center, where he led a multidisciplinary rehab team and oversaw program development, patient outcomes, and clinical operations. His background includes:

  2. Neurological conditions & fall-risk management
  3. Chronic low back, neck, and joint pain
  4. Balance, gait, and strength training for aging adults

Dr. Rivera is committed to restoring movement, independence, and quality of life through skilled hands-on care, personalized treatment plans, and a patient-centered approach. He is fluent in English and Filipino/Tagalog and is certified in Basic Life Support.

He currently treats all patients under NYC Home Rehab Physical Therapy PLLC and welcomes individuals of all ages and conditions for both home-based and clinic care.

Nicole Marie Vallejos, PTA is a licensed Physical Therapist Assistant based in Queens, NY, with extensive experience in outpatient orthopedic, geriatric, and post-surgical rehabilitation. She is currently part of the physical therapy team at NewYork-Presbyterian Hospital, where she contributes to high-level patient care within a multidisciplinary healthcare environment. Nicole is recognized for her patient-centered approach, strong communication skills, and ability to build positive therapeutic relationships that support optimal outcomes.

In addition to her hospital role, Nicole has provided care across various outpatient and long-term rehab settings, including Northwell Health STARS, Theradynamics, Yellowstone Physical Therapy, and Manjoorsa Physical Therapy. Her clinical background includes expertise in therapeutic exercise, neuromuscular re-education, strength and mobility training, balance retraining, and the use of modalities such as electrical stimulation, ultrasound, and soft tissue mobilization. She is also proficient in multiple EMR systems including WebPT, Optima, Net Health, and Clinicient.

Nicole is known for being a collaborative, motivated clinician with strong leadership qualities and a passion for helping patients improve function, independence, and quality of life. She holds an Associate of Science degree in Physical Therapist Assisting from LaGuardia Community College, where she was recognized on the Dean’s List.

Dr. Rio Jus Pedro A. Espiritu II is a licensed Doctor of Physical Therapy with extensive experience in outpatient, home health, and multidisciplinary rehabilitation settings. He specializes in the treatment of musculoskeletal, neuromuscular, and post-surgical conditions using evidence-based, function-focused physical therapy.

He has worked with a broad spectrum of patients across New York, including orthopedic injuries, chronic pain cases, sports-related conditions, geriatric mobility decline, balance and gait impairments, neurological conditions, and individuals recovering from surgery. His clinical approach emphasizes individualized care plans, targeted strengthening, mobility restoration, manual therapy, and patient education to improve independence, confidence, and long-term functional health.

Throughout his career, Dr. Espiritu has collaborated closely with chiropractors, acupuncturists, and multidisciplinary medical teams to deliver integrated, whole-person rehabilitation. He is Barbell Rehab certified, enabling him to safely incorporate strength training principles into rehabilitation for patients at all fitness levels. He is also licensed both in New York and in the Philippines and has experience utilizing multiple EMR/documentation systems across diverse clinical settings.

The Story Behind NYC Home Rehab PT

NYC Home Rehab Physical Therapy was officially established in 2025 in Astoria, New York. What began as a vision of its founder and CEO, Dr. Jerome T. Rivera Jr., Doctor of Physical Therapy, grew into a mission-driven practice built on a simple promise: to provide the highest quality one-on-one physical therapy care directly to the patient.

After years of treating patients in traditional outpatient settings, Dr. Rivera recognized a gap in the healthcare system. Many individuals, especially seniors, post-operative patients, and those with mobility challenges, could not consistently access clinic-based care. Instead of requiring patients to travel, he created a model that brings expert rehabilitation into the home.

NYC Home Rehab PT began as a solo mobile practice and expanded across Queens, Manhattan, and Brooklyn through physician referrals, patient recommendations, and a reputation for personalized, evidence-based care. The company now operates as a hybrid service, offering in-home PT, outpatient clinic visits, and telehealth, while remaining rooted in its founding belief:
