He specializes in helping athletes and active adults recover from injury, improve performance, and return safely to sport—with a particular focus on sprinting mechanics, deceleration control, and jump-based power training.

Carlo has treated a wide range of orthopedic and post-operative conditions in both clinic and private settings, and has worked with competitive soccer athletes through the MLS Next Academy at Metropolitan Oval. His background includes experience at Motion Sports Medicine, Fit Club Astoria, US Athletic Training Center, and as owner of a private PT practice. He has also rehabilitated Olympic weightlifters and high-performance athletes, integrating strength training with evidence-based rehab.

Carlo is currently completing his Transitional Doctorate in Physical Therapy (tDPT) at the University of Montana and holds a Bachelor of Science in Physical Therapy from the Philippines.

Alongside his clinical work, Dr. Baldeviso has served as a national board exam lecturer for physical therapy students, teaching anatomy, kinesiology, and advanced rehabilitation concepts. He holds multiple specialty certifications including geriatrics, stroke rehab, Astym soft-tissue therapy, spine care, and cardiac rehabilitation, and is CPR/BLS certified.

She earned her Bachelor of Science in Physical Therapy from De La Salle Health Sciences Institute in Cavite, Philippines. She brings over eight years of clinical and leadership experience across rehabilitation clinics, fitness-based therapy settings, and academic institutions, allowing her to integrate clinical precision with effective communication and team collaboration.

She currently practices as a full-time Physical Therapist in an outpatient rehabilitation clinic in New York, where she treats a broad range of neuromusculoskeletal, sports injury, and post-surgical cases. Her treatment approach incorporates therapeutic exercise, manual therapy, physical modalities, and structured progression based on patient goals and functional demands. She is proficient in electronic medical record documentation and experienced in coordinating care with insurance providers and managed care organizations.

Her experience includes treating patients with conditions such as stroke (CVA), spinal cord injury, Parkinson’s disease, cardiac conditions, cancer-related deconditioning, cerebral palsy, and generalized weakness. He is the founder of NYC Home Rehab Physical Therapy PLLC, a hybrid practice providing one-on-one physical therapy both in-clinic and through mobile in-home visits across New York City.

Dr. Rivera earned his Doctor of Physical Therapy from the University of Montana and graduated cum laude with a Bachelor of Science in Physical Therapy from the University of Santo Tomas in the Philippines. He is licensed in New York and previously practiced in the Philippines, where he gained clinical experience in hospital-based, outpatient, pediatric, and cardiac rehabilitation settings.

Before launching NYC Home Rehab Physical Therapy, Dr. Rivera served as a Director of Physical Therapy in an outpatient medical center, where he led a multidisciplinary rehab team and oversaw program development, patient outcomes, and clinical operations. His background includes:

NYC Home Rehab Physical Therapy was officially established in 2025 in Astoria, New York. What began as a vision of its founder and CEO, Dr. Jerome T. Rivera Jr., Doctor of Physical Therapy, grew into a mission-driven practice built on a simple promise: to provide the highest quality one-on-one physical therapy care directly to the patient.
